- Iris damage due to acute glaucoma
- Mydriatic drugs, eg, atropine
- Sympathomimetic poisoning, eg, amphetamines, cocaine
- Anticholinergic poisoning, eg, atropine, tricyclic antidepressants
- Physiologic anisocoria (unequal pupils, react normally)
