  • Cranial nerve 3 palsy
  • Iris damage due to acute glaucoma
  • Mydriatic drugs, eg, atropine
  • Sympathomimetic poisoning, eg, amphetamines, cocaine
  • Anticholinergic poisoning, eg, atropine, tricyclic antidepressants
  • Physiologic anisocoria (unequal pupils, react normally)
