- Mycoplasma infection
- Peritonsillar abscess

- Chronic fatigue syndrome
- Viral hepatitis
- Toxoplasmosis
- Rubella
- Acute leukemia or lymphoma
- Pertussis (both may have atypical lymphocytosis)
- Hypersensitivity reaction to carbamazepine
