- Cranial nerve 3 palsy (dilated or normal pupil), eg, diabetic neuropathy, posterior inferior cerebellar artery (PICA) aneurysm
- Horner’s syndrome (constricted pupil)
- Myasthenia gravis
- Botulism
- Age-related (sagging eyelid)
- Eyelid inflammation or deformity
- Congenital
- Progressive external ophthalmoplegia (mitochondrial myopathy)
