  • Chronic obstructive lung disease or airway obstruction
  • CNS depression: opioids, O2 in CO2 retainer
  • Neuromuscular, eg, Guillain-Barré, myasthenia gravis, botulism
  • Structural disorder of the thorax
  • Myxedema
