Etiology
- Crush injury
- Prolonged immobility, eg, drug overdose, exposure hypothermia
- Statin use
- Myositis, eg, polymyositis, dermatomyositis
- Seizure
- Strenuous exercise or heat stroke
- Hypokalemia or hypophosphatemia
- Severe volume contraction
- Influenza
