Dedication

Peter Rosen, MD has always been a larger-than-life figure and many regard him as the “father” of emergency medicine. He has been a mentor, teacher, inspiration, and friend to all of us, but perhaps most important has been his commitment to family as husband, father, grandfather, and uncle. His charisma, enthusiasm, and love of life are obvious in every encounter and provide all of us with a perspective that is fundamental to the love of our specialty. Peter taught us how to care for the most difficult patients who present for treatment in the emergency department.

Originally trained as a general surgeon serving in the Army and later practicing in Wyoming, he subsequently worked at the University of Chicago where he started the residency in emergency medicine. Moving to Denver as the Chief of Emergency Medicine until 1989, Peter created an environment that stimulated learning, excellence in clinical care, and leadership. Thanks to Peter’s vision, the Denver program was the home of many of the pioneers of emergency medicine during the last three decades. The University of California, San Diego then recruited him to serve as a mentor and role model for the next generation of students of emergency medicine. Beyond the clinical setting, Peter could always be found skiing, playing tennis, golfing, and sailing with abandon.

During his career he has been the editor of many of the world’s leading emergency medicine textbooks and journals. Peter has served on many academic societies including the Board of Directors of the American Board of Emergency Medicine during its formative years and has been recognized for his outstanding contributions and leadership by the American College of Emergency Medicine and the Society of Academic Emergency Medicine.

Peter’s imprint upon emergency medicine has been instrumental in the growth of our specialty and the advancement of excellence in the care of our patients in the emergency department setting.

It is with incredible reverence, respect, and love that we dedicate this edition of Rosen & Barkin’s 5-Minute Emergency Medicine to Peter. Peter conceptualized this book in our initial discussions and has been a force in allowing us to continue its evolution.

Thank you Peter for your dedication to colleagues, students, and patients over nearly five decades. All of us have benefitted from your wisdom and insight and will forever be indebted to you for your mentorship during our growth as clinicians and teachers.

Preface

Rosen & Barkin’s 5-Minute Emergency Medicine Consult has evolved to reflect the unique quality of our practice of emergency medicine. Our clinical environment provides challenges for the clinician; the remarkable breadth of clinical conditions encountered, the time constraints of an acute illness, environmental considerations, and the logistical demands of busy emergency departments (EDs). Time is of the essence! This book is designed to meet the needs of clinicians working in settings providing urgent and emergent care. To look for a diagnosis, one must think of it, and the very nature of the ED makes prolonged deliberation difficult. Nevertheless, it must become instinctive to think about the statistically rare, but clinically serious entity, rather than to just reach for the statistically probable, but non–life-threatening diagnosis.

The concise format allows the busy clinician to respond appropriately to each patient. It is meant to be readily available and frequently used in the trenches of the ED. Written and edited by practicing clinicians for our colleagues, the book synthesizes a vast body of information into tightly formatted chapters that stimulate analysis and subsequent assessment.

We have attempted to integrate our authors’ expertise, experience, and knowledge base onto the pages of this book. The chapters are not meant to be a diagnostic engine, but rather a place to confirm a diagnosis supported by clinical judgment and the subjective and objective evidence of a patient’s presentation. Our authors provide a precise and clinically relevant summary useful in caringfor the patient while also equipping students and residents with the structure needed to approach individual disease processes. We are indebted to our contributors for their commitment to this task.

The book is intended to be readily integrated into practice, rather than being definitive and all encompassing. As in the past, this new edition incorporates new information and approaches to management, while allowing us to modify topics that reflect some of the new challenges we face.

Our goal is to make Rosen & Barkin’s 5-Minute Emergency Medicine Consult useful to both novices and experienced clinicians in emergency medicine. The information and organization is designed to be easily used within the “chaos” that surrounds our clinical settings.

Clinical acumen, judgment, and experience are the foundations for our clinical practice. It is our hope that this book will serve practicing emergency physicians, nurses, physician assistants, emergency medical personnel, and students as a readily used resource in facilitating excellence in our care of our patients who will always remain the focus of our efforts, while advancing the specialty of emergency medicine.
